As we age, many of us need some help in order to continue to live independently. Without this crucial assistance, seniors often face being forced to give up their cherished independence for premature institutionalization.
WISE Care Management offers a variety of services to
ease the burdens faced by frail/elderly seniors and
adults with disabilities who require some assistance
with daily living in their own homes, in order to continue
living independently. Often, concerned family members
want to help their loved one but don't know what resources
are available.
Our professional Care Managers begin with an in-depth assessment in the home of each individual's specific needs and create a personalized plan of care. Many of our clients may be coping with low incomes, chronic medical conditions and social isolation. Our Care Managers then advocate on their behalf to ensure they get the necessary assistance they need to improve their self-sufficiency and their quality of life.
This highly effective program exists to enable the frail/elderly and adults with disabilities to live safely in their own homes and improve their quality of life. Fifty percent (50%) of those served are between 80 and 100 years old; 70% have some disability; and almost all have a high degree of frailty. Over 90% of the clients served are able to continue to live independently in the community.
