WISE Adult Day Service Center is seeking a candidate for a Activity Leader position. The goal of the Adult Day Service Center is to provide recreational, educational, and therapeutic activities for frail elderly and Alzheimer's patients. The successful candidate will participate in the individualized care planning process for program participants and assist with personal care needs of clients including escorting participants to the bathroom and providing hygienic care. The Activity Leader will also plan, prepare and implement program activities as assigned, provide a clean, safe, orderly, and attractive environment for all participants and assist with meal and snack preparation.

Requires Certified Nurse’s Assistant certificate and minimum of 2 years working with the cognitively-impaired and disabled elderly (preferably in an adult day care program). One year previous experience working in the field of social services. High School diploma required. Excellent written and oral communication skills. Must have strong interpersonal and demonstrated teamwork skills.

Upon hire, the successful candidate will obtain a Health Screening report, including proof of tuberculosis and background clearance.
